Box 2089 Detroit Mi. 48231-2089 Telephone number is 1-800-482-5146 On December 14, 2007 an Order placing National Foot Care Program, Inc into liquidation was entered. The Order of Liquidation was entered because, among other reasons, National Foot Care could not be successfully rehabilitated due to its financial condition. Healthcare Vendor Management has been in contact with representatives within the Office of Financial and Insurance Services (OFIS), State of Michigan regarding outstanding claims and threats of collection being received by Ford members. OFIS advises that Liquidation Notices and Proof of Claims Forms are being mails to all members who were enrolled in NFCP coverage (one per household to last address on NFCP's records) and all providers (in all states, not just Michigan). The mailing of these documents has begun and, due to the large volume, is to be completed by 1/14/2008. Members who have received National Foot Care related bills from providers can submit same via utilization of the Proof of Claims form which can also be obtained by accessing the following website and clicking on 'National Foot Care Program': Mailing and FAX information for submittal of bills is indicated on the Proof of Claim Form.
All pertinent documents regarding the NFCP liquidation can be found on the above website. Attached are copies of the 'Notice of Court Order to Liquidate', 'Proof of Claim Instruction's and 'Proof of Claim Form' documents for your reference. In preparation for the actual liquidation procedure, OFIS is processing claims but not issuing actual payments to providers at this time. In Michigan, providers are prohibited by law from billing National Foot Care members directly for services rendered. In accordance with the Order of Liquidation: "all non-contracted and contracted medical care providers are enjoined from pursuing collection against, obtaining judgment against, and/or balance billing of National Foot's subscribers, enrollees or members for medical good provided or services rendered prior to the date that this Liquidation Order is entered." All claims against National Foot Care Program must be filed in the Ingham County liquidation proceeding in accordance with the Proof of Claim Instructions. The attached Notice of Court Order to Liquidate includes this verbiage and members can direct providers to this document. We have been advised that there may be some issues with providers outside of the state of Michigan. If the provider participated and signed a contract with NFCP there is a hold harmless clause in the contract which prohibits the provider from balance billing the member for covered services. There also seems to be some confusion on the non-network provider benefit level and the amount the member can legitimately be billed for. If the member choose to received services from a non-participating provider, foot and ankle care expenses are payable at 50% of the established NFCP prevailing rate or 50% of the actual change to the member, whichever is less. Reimbursement will not be made on services rendered by a non-participating provider for services determined to be not medically necessary. In addition, certain services received from a non-participating provider, such as office visits, physical therapy and orthotics are not covered at all under the National Foot Care Program. Claims Procedure and Claims Bar Date The Order of Liquidation, as provided by MCL 500.8122, requires that notice be given to all persons or entities known or reasonably expected to have claims against National Foot that were incurred on or before 12/14/2007 and for which the liquidation estate may be liable. IMPORTANT: If you have such a claim and wish to be considered for payment of this claim in the liquidation proceedings, you must file a proof of claim with National Foot that complies with the requirements of MCL 500.8136. Failure to file a completed Proof of Claim form complying with these requirements and/or filing a Proof of Claim form postmarked after the Proof of Claim deadline of June 14, 2008 (the bar date) will result in your claim being disallowed.
